The pharmaceutical landscape in India is bracing for a significant transformation as the patent for semaglutide, held by the Denmark-based Novo Nordisk A/S, expires this week. This milestone is expected to unleash a flood of affordable generic alternatives for diabetes and weight-loss management, potentially reshaping one of the world's most populous healthcare markets. More than 40 local pharmaceutical companies are preparing to introduce over 50 brands within weeks, widening access in a price-sensitive market while raising concerns about potential misuse. Key players expected to enter the fray include Mankind Pharma Limited, Zydus Lifesciences Limited, Lupin Limited, and Alkem Laboratories Limited, alongside other major manufacturers like Sun Pharma and Dr. Reddy's. These firms aim to provide generic versions of the active ingredient found in popular treatments like Ozempic and Wegovy. Salil Kallianpur, an independent analyst, noted the risks associated with such a rapid expansion. > With high demand, falling prices and multiple brands, you may see direct pharmacy purchases, distributor-level leakages, or cosmetic or lifestyle use especially in urban markets. The arrival of these generics will also heighten competition for major players from the United States, including Eli Lilly and Company, which has seen significant success with its own diabetes and obesity treatments. India currently holds the second-highest number of adults living with diabetes globally, trailing only China, and researchers estimate the country could have more than 440 million overweight or obese individuals by 2050. Generic drugmakers in the region are expected to offer their products at discounts of at least 50% to 60%. Market analysts suggest that monthly costs for low-dose treatments could drop from 11,000 rupees to between 1,500 and 2,500 rupees as the market matures, making the medication accessible beyond a small urban elite. Vishal, a tech worker from Hyderabad, expressed interest in the new options. > I will consult my doctor to check if I can move to using a generic version, as that appears to be lighter on the pocket. Despite the drop in prices, the long-term leaders in the market will likely be determined by physician confidence rather than cost alone. Experts believe that the initial surplus of products might overwhelm healthcare providers, leading to a period where only a few companies with high-quality delivery systems and consistent outcomes maintain their market share. Kallianpur added that the market will eventually consolidate. > Weaker players with poor quality and no differentiation will likely exit within two to three years.
